Medical cannabis for neuropathic pain

Neuropathic pain is often refractory to conventional agents. Cannabinoids are studied as an option in selected, treatment-resistant cases.

The clinical problem

Neuropathic pain — from diabetic neuropathy, post-herpetic neuralgia, nerve injury and other causes — responds incompletely to many conventional analgesics, and first-line agents are often limited by tolerability. Refractory cases are common.

Rationale for cannabinoids

The endocannabinoid system modulates nociceptive processing at multiple levels, providing a mechanistic rationale for cannabinoids in neuropathic pain that differs from existing agents. They are generally considered in selected, treatment-resistant patients rather than first line.

What the evidence says

Trials are heterogeneous in population, product and outcome; effect sizes, where positive, are typically modest and balanced against CNS adverse effects. Appraise the source-linked studies in the clinical evidence library for the specific neuropathic context you are treating.

Practical considerations

Reserve for selected refractory cases, individualise the CBD:THC ratio, and titrate slowly (see the dosage guide). Check interactions and counsel on adverse effects and driving. Set realistic, function-focused goals and review objectively.
